Support MQSAMP_USER_ID for MQ High Availability Samples

We are running the EUS version of MQ on CP4I 2020.4.1 on Openshift 4.6.53 on Azure cloud.

The MQ High Availability Sample programs such as "amqsphac" & "amqsghac" on MQ 9.2 do not prompt a password even after "MQSAMP_USER_ID" environment variable is set.


We have a security policy within the company which requires us to enforce username/password based authentication for communication to MQ QMGRs, so lack of this feature makes it very difficult for us to do HA tests.


Additional environment variables set:

    You could use a CCDT instead of the MQSERVER env variable. In the CCDT specify the mqccred exit on the CLNTCONN, this will then pass the username and password for you.
